Endoscopists of the Republican Oncology Center have restored a patient's breathing for the first time in Tatarstan by using a Y-shaped stent.
When a tumor simultaneously affects the trachea and both main bronchi, a standard linear stent can no longer ensure airway patency. That is why the endoscopists of the Republican Clinical Oncology Dispensary named after Prof. M.Z. Sigal performed the installation of a Y-shaped bifurcation stent for the first time in Tatarstan — a special metal mesh structure that helps restore airway patency. Due to its shape, such a stent allows for the simultaneous restoration of the trachea and both main bronchi. The patient was admitted to the dispensary with severe shortness of breath. Without oxygen support, the blood oxygen saturation level dropped to 80%. Computed tomography revealed a malignant lung tumor with metastases to the intrathoracic lymph nodes. They had invaded the lower third of the trachea and both main bronchi, narrowing the airway lumen by more than two-thirds. During bronchoscopy, doctors confirmed critical narrowing of the trachea and bronchi, as well as finally determined the volume of the upcoming intervention. Preparing for the operation required careful planning. Based on the data from computed tomography and bronchoscopy, specialists individually selected the model and size of the Y-shaped bifurcation stent, taking into account the patient's anatomical features. The intervention was performed under intravenous anesthesia. Despite the high complexity of the case and the fact that such a technique had not been used in Tatarstan before, the doctors successfully installed the stent and completely restored the patency of the trachea and both main bronchi. Just a few hours after the operation, the patient was transferred from intensive care to the thoracic department No. 1. He was breathing independently without oxygen support, and a few days later was discharged home.
